Membership Agreement

Firestorm Services
- MyFirestorm is an affiliation of networking groups whose objective is to facilitate and provide opportunities for the growth and development of professional relationships.
- In order to further that growth and development, Firestorm will provide to those members exclusive access to ongoing education, seminars, web listings, and other member benefits. Use of such benefits are solely at member’s discretion.

Term of Membership
- Membership is a one year term at a rate of $30.00 per month. If paid annually, a discounted rate of $330 per year shall be accepted.
- Membership shall run from the time the member completes payment, i.e., if you fill out your payment information on the 14th of the month you will be billed on the 14th of the month going forward. Membership is automatically renewed at the beginning of each annual anniversary unless terminated by written notice thirty days in advance.

Payment and Dues
- Payment of dues may be by check if they are annual, or by credit card for monthly / annually. Yearly payment made by check should be payable to MyFirestorm, LLC and sent to:

- Dues are to be paid by the day on which they are owing. If a card is declined we will contact the member to determine the issue. Member will be provided a written warning for the first offense. The second offense will result in a request by Firestorm to Member to terminate membership.
- Should dues payment be returned for insufficient funds, Member will pay the late dues as well as NSF fees charged to Firestorm and membership will be immediately terminated.

Membership Responsibilities and Rules
- Members may have one professional category per chapter. If member wishes to represent additional categories, written permission from the chapter president must be obtained and member will pay an additional membership fee for each additional category. Attendance and participation are key factors to the success of the group. Networking group members can participate by attending meetings, proactively providing leads and referrals to members, and by actively building their business relationships.
- Members agree to follow-up on referrals given them. Members agree to share information and market the group as an overall goal of membership.

Termination
- Firestorm retains the right to terminate membership, without refund, if Member fails to provide accurate, complete, and up to date information, violates any terms of this agreement, or other just cause as Firestorm determines appropriate.
- Individual chapters may terminate Members membership for the following reasons:
- Poor attendance – attendance policies are held by the individual chapter.
- Lack of referral passing
- Lack of participation
- Ethical considerations
- Member may additionally be terminated upon receipt of a complaint and investigation of said complaint by MyFirestorm in conjunction with the Chapter President. Member shall have the right to receive notice of said complaint in writing and have the opportunity to respond in writing prior to a determination of membership by MyFirestorm. All decisions of Firestorm are final.
- If member wishes to terminate this agreement prior to the end of their year, the balance of the membership dues will be due upon termination, i.e., if you complete the payment process on 1/1/11 your agreement goes through 12/31/11. If you wish to terminate the agreement on 8/1/11 the balance of the remaining 5 months, or $150, is due on 8/1/11. If you've paid for the year in advance there will be no refunds given. To terminate this agreement written notice must be sent 30 days in advance to the address above in section 2.
